1.         The Promoter

1.1       This campaign promotor of this campaign is Abbott Laboratories Ireland Limited (registered company number 11542) whose registered office is Block B, Liffey Valley Office Campus, Quarryvale, Dublin 22, D22 X0Y3, Ireland (VAT no. IE8P56068T).

2.         The Campaign and Eligibility  

2.1       The title of the campaign is IE: Self-funded Sampling Campaign and eligible individuals can receive one (1) free of charge FreeStyle Libre 2 Sensor in accordance with these terms and conditions.

2.2       The campaign is open to:

(a)   people with diabetes aged 18 years or over who reside in the Republic of Ireland. Under 16s cannot participate in this campaign; this campaign is also open to parents/guardians of children with diabetes under the age of 18 and over the age of four;

(b)   people who have not used or trialled the FreeStyle Libre system or the FreeStyle Libre 2 system; and

(c)   people who are not eligible to receive the FreeStyle Libre 2 Sensor on prescription.

2.3       In entering this campaign, you confirm that you are eligible to do so and eligible to claim one (1) free of charge FreeStyle Libre 2 Sensor. Abbott may require you to provide proof that you are eligible to enter the campaign.

2.4       Prior to entering, you should ensure that you have read and understood the purposes for which the FreeStyle Libre 2 System should be used. This information is available on the FreeStyle Libre website.

2.5       There is a limit of one (1) entry per person. Entries on behalf of another person will not be accepted and joint submissions are not allowed.

3.         How to Enter

3.1       The campaign will open from 00.01am on 12th July 2023 (“Opening Date”). Abbott will notify users on its website of any closing date. Entries which are made prior to the Opening Date will not be eligible to participate in the campaign.

3.2       In order to receive one (1) free of charge FreeStyle Libre 2 Sensor, please complete the details and the diabetes eligibility questionnaire set out in the Application Form on the FreeStyle Libre 2 Sampling page. 

3.3       Once the application form is submitted, Abbott will check your details against Abbott’s existing databases in line with the Privacy Policy to confirm that you are a new customer. Answers to the diabetes eligibility questionnaire in the Application Form will need to be answered correctly to qualify for one (1) free of charge FreeStyle Libre 2 Sensor. Once validation is completed by Abbott, you will receive a voucher code to the email address that you provide on the Application Form to be used on the FreeStyle Libre 2 Sampling page to redeem one (1) FreeStyle Libre 2 Sensor free of charge.

3.4       No purchase is necessary and there is no charge to register for use of the website or for postage and packaging of the free sample.

3.5       Abbott accepts no responsibility for Application Forms that are lost, mislaid, damaged or delayed in transit, regardless of cause, including, for example, equipment failure, technical malfunction, systems, satellite, network, server, computer hardware or software failure of any kind.  

3.6       By submitting an Application Form, you are agreeing to be bound by these terms and conditions.

3.7       You confirm that if you are eligible for one (1) free of charge FreeStyle Libre 2 Sensor, it shall not be sold, billed, traded, charged or subject to any claim for reimbursement to any individual or company for a monetary or any other value.

3.8       There will be no cash alternative or prize alternative, other than as set out in paragraph 3.9.

3.9       In the event that all or part of any prize is unavailable, Abbott reserves the right to offer an alternative prize of equal or greater value.

3.10    Abbott is not liable for any disruption to this campaign for reasons beyond Abbott’s reasonable control. In the event of any disruption to this campaign, Abbott will use its reasonable endeavours to resolve the disruption and resume the campaign in a manner that is fair to the participants.

3.11    Abbott reserves the right to delay the timing of notification.

3.12    By entering the campaign, you agree to be bound by these terms and conditions. Any entries not complying with these terms and conditions may at Abbott’s discretion, acting reasonably, be deemed invalid and disqualified.

4.         Data Privacy

4.1       You agree that your personal data supplied during the course of this campaign may be passed on to third party suppliers solely for the purpose of this campaign. Please see our privacy policy if you have any questions.

5.         General

5.1       If there is any reason to believe that there has been a breach of these terms and conditions, Abbott may, at its sole discretion, reserve the right to exclude you from participating in the campaign.

5.2       Abbott may disqualify any entrant who it reasonably believes has interfered with or attempted to interfere with the fair running of this campaign.

5.3       Abbott reserves the right to hold void, cancel, suspend, or amend the campaign where it becomes necessary to do so.

5.4      This campaign is governed by the laws of the Republic of Ireland and users submit to the exclusive jurisdiction of the courts of the Republic of Ireland.
