1. The Promoter
1.1 This campaign promotor of this campaign is Abbott Laboratories Limited, a company registered in England and Wales with company registration number 329102 and registered office address at Abbott House, Vanwall Business Park, Vanwall Road, Maidenhead, Berkshire, SL6 4XE.
2. The Campaign and Eligibility
1.1 The title of the campaign is Ireland: Self-funded Sampling Campaign.
1.2 The campaign is open to:
a. people with diabetes aged 22 years or over who reside in Ireland. Under 22s cannot participate in this campaign; and
b. people who have not used or trialled the FreeStyle Libre system.
1.3 In entering this campaign, you confirm that you are eligible to do so and eligible to claim one (1) free of charge FreeStyle Libre sensor. Abbott may require you to provide proof that you are eligible to enter the campaign
1.4 Prior to entering, you should ensure that you have read and understood the purposes for which the FreeStyle Libre System should be used. This information is available on the FreeStyle Libre website (www.FreestyleLibre.ie).
1.5 There is a limit of one (1) entry per person. Entries on behalf of another person will not be accepted and joint submissions are not allowed.
3. How to Enter
1.1 The campaign will open from 00.01am on 14th November 2020 (“Opening Date”). Abbott will notify users on its website of any closing date. Entries which are made prior to the Opening Date will not be eligible to participate in the campaign.
1.2 In order to receive one (1) free of charge FreeStyle Libre sensor, please complete the details and the diabetes eligibility questionnaire set out in the Application Form [https://sample.freestyle.abbott/ie-en/freestylelibre.html].
1.3 Once the application form is submitted, Abbott will check your details against Abbott’s existing databases in line with the Privacy Policy to confirm that you are a new customer. Answers to the diabetes eligibility questionnaire in the Application Form will need to be answered correctly to qualify for one (1) free of charge FreeStyle Libre sensor. Once validation is completed by Abbott, you will receive a voucher code to the email address that you provide on the Application Form to be used on www.FreeStyleLibre.ie to redeem one (1) FreeStyle Libre sensor free of charge.
1.4 No purchase is necessary and there is no charge to register for use of the website.
1.5 Abbott accepts no responsibility for Application Forms that are lost, mislaid, damaged or delayed in transit, regardless of cause, including, for example, equipment failure, technical malfunction, systems, satellite, network, server, computer hardware or software failure of any kind.
1.6 By submitting an Application Form, you are agreeing to be bound by these terms and conditions.
1.7 You confirm that if you are eligible for one (1) free of charge FreeStyle Libre sensor, it shall not be sold, billed, traded, charged or subject to any claim for reimbursement to any individual or company for a monetary or any other value.
1.8 There will be no cash alternative or prize alternative, other than as set out in paragraph 3.9.
1.9 In the event that all or part of any prize is unavailable, Abbott reserves the right to offer an alternative prize of equal or greater value.
1.10 Abbott is not liable for any disruption to this campaign for reasons beyond Abbott’s reasonable control. In the event of any disruption to this campaign, Abbott will use its reasonable endeavours to resolve the disruption and resume the campaign in a manner that is fair to the participants.
1.11 Abbott reserves the right to delay the timing of notification.
1.12 By entering the campaign, you agree to be bound by these terms and conditions. Any entries not complying with these terms and conditions may at Abbott’s discretion, acting reasonably, be deemed invalid and disqualified.

5. General
1.1 If there is any reason to believe that there has been a breach of these terms and conditions, Abbott may, at its sole discretion, reserve the right to exclude you from participating in the campaign.
1.2 Abbott may disqualify any entrant who it reasonably believes has interfered with or attempted to interfere with the fair running of this campaign.
1.3 Abbott reserves the right to hold void, cancel, suspend, or amend the campaign where it becomes necessary to do so.
1.4 This campaign is governed by the laws of Ireland and users submit to the exclusive jurisdiction of the courts of Ireland.
